If you prefer, you may print the Mortgage Questionnaire and mail it in (or drop it off) and pay your $30 processing fee with a money order. You may also pick up a copy of the Mortgage Questionnaire at our office at 9333 N Normandale St, Fort Worth, TX 76116.
If completing the questionnaire online, you must pay your $30 processing fee by credit/debit card.
